Grades 6–12:
Contact the appropriate Middle School or High School Guidance Office to schedule an appointment.

Child’s birth certificate
Current immunization records
Proof of residency
Completed registration packet
Custody documentation, if applicable
For students receiving special education services: most recent IEP, NOREP, and evaluation/re-evaluation reports
